Contacting Family Support Services Staff

There are two ways to contact the Family Support Services staff at the centers.  One is to request a referral form from your child’s teacher, center director, or center clerk.  The second way is for your child’s teacher to make a referral for you and/ or child.  When completing the referral form, please make sure to include the following:

  • Your name and best contact number

  • Your child’s name and teacher’s name

  • A short description of reason for referral

Completed referral forms must be returned to the child’s teacher, center director, or center clerk.  Center directors review all referrals before submitting to FSS and someone from the FSS office will contact you within 7 days of making the referral.

For any immediate family support assistance contact the center director.
